  • Results in 10 minutes

• Enrollment: $50
• Monthly membership: $150
• One-time office/home visit: $300
• One-time telemedicine visit: $175
• Home visit: $300 (on top of visit fee) (2 free/year)
• Less than 24 hour cancellation or missed appointment: $50
• If you join before or on the 15th of the month, you will get charged the full amount of the month, sign-up after the 15th and it will be half of the monthly fee for that month.
• Any fees associated with late payments will be the patient’s responsibility
• Membership paid up front 3, 6, or 12 months will be discounted.

Additional Fees:

• Any office examination for any complaint not related to your annual or yearly preventative exam will be $30.
• If a vaccine or injection is warranted during an office visit, additional fee of $30.
• The patient is responsible for the cost of any IUDs and a fee of $75 for insertion of the IUD.
• Re-enrollment: $75
